A patient who has been suffering from left hip pain for the past nine months consulted our iCliniq doctor. He said that he took an MRI (magnetic resonance imaging) of the hip, and it showed mild synovitis (swelling of the tissue that lines the inside of a joint) of the left hip. He also mentioned that he had been taking medication for chest TB for the past three years. He added that he had recently consulted a doctor who advised him to switch to Akurit 4 (Isoniazid, Rifampicin, Ethambutol, and Pyrazinamide) for bone tuberculosis (TB). The patient wanted to know if it was actually bone TB.

Our Pulmonologist went through the query and said that as the patient previously had chest TB, the MRI finding is more likely to be TB of the hip. The doctor also asked the patient to get ESR (erythrocyte sedimentation rate) and CRP (C-reactive protein) tests done as they might help to monitor the treatment response.

The patient reverted with test reports and asked the doctor whether switching from Akurit 4 to Akurit 3 (Rifampicin, Ethambutol, Isoniazid) before the prescribed duration might create any problem. He also wanted to know the duration of Akurit 4 and Akurit 3 in bone TB treatment.

Our doctor went through the query and said that Akurit 4 could be given for a maximum of three months, followed by a three-drug combination, the Akurit 3. The doctor also asked the patient to take Akurit 4 for now and said that the significant amount of hip pain would be reduced, but a complete reduction of pain can take up to one year.

After one year of medication for bone TB, the patient reverted, saying that he had very little pain and only felt a little discomfort in his hip while walking without any support, and it has gradually reduced. The patient added that he had recently done an MRI and LFT (liver function test) and asked the doctor about the next course of treatment.

The doctor went through the test reports and said that the disease activity had been significantly reduced. So he asked the patient to stop the medicine and start some hip and back exercises to get back the mobility in the muscles.
