Medical Case Details:

Hello Respected Doctors,
A 9 month old female baby came to me with red-pink spot on her right cheek. Her mother observed it first time when she was 10 days after birth. She was scared about it. I diagnosed this as a capillary haemangioma and advised her to do nothing and it'll subside itself at the age of school going. But I need expert opinion about this. What should be the diagnosis and the medications?

    The diagnosis is Hemangioma

    looks like it is growing in size

    this size hemangioma at this cosmetic site should be treated at the earliest

    refer the baby to a Dermatologist , they will treat it accordingly( options are lasers and medications)

    It is a Hemangioma.

    These days "to do nothing" is not appropriate in such a big lesion in a prominent cosmetically important location. Early treatment was required in this case. Already too big.

    Treatment is Propanolol trial >> Surgery / Laser ablation. Do not start medication if you cannot followup and monitor the baby. Best is Refer to Pediatric Dermatology in AIIMS.

    Dr. Ishwar above mention patient is a case of capillary hemangioma.
    treatment is only reassurance patient parent about natural history of Disease. it grow till age of 2 year and then subside gradually like 50% at age of 5 year and 70% at age of 7 year. As lesion present on nonvital structure so no active intervention required. you can start propanolol @ 2mg/kg wt of pt in divided dose to fasten remission. After remission surgery may be needed to remove fibrofatty tissue.

    I agree with Dr. Manish Jain. The lesion is on the cheek and as it is not affecting any functions of the child, the vision, hearing, speech, feeding etc, its always better to wait and follow up. This will regress on its own by 5-7 years. Proper counselling of the parents is necessary and are asked to avoid any trauma in this region. Propranalol treatment is also not fully indicated considering the risk/benefit ratio. But after the regression of the lesion, an intervention in the form of laser or surgical excision might be required for any remnants.
