Welcome to icliniq! As you have already mentioned, it seems to be fungal infection. We don't know ayurvedic medicines but in allopathy, we advise tablet fluconazole 200 mg once daily for 7 days. His HB, diabetes profile should be checked. Also one needs to rule out sexually transmitted diseases along with HIV. Give dose of vitamin and 2gms of secnidazole.

Hello Doctor, As per the history it seems to be a fungal infection, Tinea cruris. Extreme itching, flakiness and scaling are typical of fungal infection and lesions may have a well-circumscribed appearance as well. Management:
    1. Ruling out any immunocompromised states like HIV, DM, Dietary deficiencies. Excluding contact dermatitis.
    2. Keeping area clean and towel dried frequently. Ensure wearing loose clothing. Advice on cessation of urge to scratch to prevent any secondary infections.
    3. Medications: -Ketoconazole soap to clean and wash area.
    - Abzorb powder to dust over affected part TID
    - Tab. fluconazole 150mg per week for 4 weeks
    - Anti-histamines to prevent itching and vitamin supplementation would also help
    4. Treating close contacts of patient is important and advice not to share towel or personal belonging with others.
I hope this is helpful. Thank you.
