Medical Case Details:

If you are managing a 56 year old woman with severe pneumonia in the Intensive Care Unit, what will you do when you are called to see her because her leg has suddenly turned blue?

Please tell me the diagnosis and management.

    Bedside Compression USG to look for DVT in calf as well as femoral veins. DVT prophylsis or therapy as the case may be.

    DVT, is there calf tenderness?
    Peripheral pulses? If absent? May peripheral some vein is blocked,
    Emergency Doppler study, angiography if possible to confirm.
    Bed rest. Do not stand and walk till thrombas stabilised.
    IV fluids, oxigen, leg resting position with pillow.
    Inj flexiparum low molecular heparin 2000 units subcutaneous daily if confirm diagnosis.
    Inj heparin 5000 units s/c with control of PT.
    Or IV infusion of heparin
    When colour normal shift to aspirin or clopidogril and gentle movent Chang possible, start walk gradual.

    Hi, We must check for any line of demarcation of temperature over the skin of that leg, any absence of peripheral pulse. one should start LMWH as soon as possible in this case. And proceed to investigate with arterial Doppler of that leg.
