Medical Case Details:

Male, 26 years, complains of severe weakness, headache, and pain in the left shoulder (could not move the left hand), after a traffic accident. There is a bleeding wound on the left shoulder. What is the primary diagnosis? Please suggest emergency management.

    Based on the patient’s history and symptoms, I would consider the possibility of Brachial plexus injury on the left shoulder and Whiplash injury. I would recommend an MRI of the cervical spine and the Left shoulder and an EMG-NCV of Both UE.

    As you mentioned pt was having shoulder injury and patient is unable to move hand . It could be a brachial plexus injury. Kindly send patient to a plastic surgeon who is doing brachial plexus surgery.

    I think, clinical history need to be elaborated a bit more:-
    1) patient has pain in left shoulder and unable to move his shoulder, the likely cause of inability to move the shoulder is pain , so I will rule out the local cause first which includes shoulder dislocation and shoulder joint injury, shoulder bone injury.
    2) brachial plexus injury can also have similar presentation, however other cause should also be taken into consideration.

    Severe weakness may be due to blood loss. Take a proper history and vitals. He may need IV or blood transfusion. Proper examination is needed to rule out fracture or dislocation.
