Should we start any other preventive treatment? (Internal Medicine)

Medical Case Details:


My patient 81 years old had appendix and sigmoid resection due to moderately differentiated adenocarcinoma pT2N1Mx with a few metastasis in nearby lymph nodes, no other metastasis.

Should we start any preventive treatment like chemotherapy or radiation?

    According to the general condition the cardiac condition is the most important as all chemo therapy is cardio-toxic

    No diabetes, liver fine, on medication for HTN only: diuretic and ARB.

    You can start treatment with very close follow up to the cardiac and liver function. If any indication of liver or cardiac affection you will stop and do a close follow.

    Yes, basically, your patient requires adjuvant chemotherapy for stage III Sigmoid colon cancer based on a clinical trial results. Chemotherapy arm patients did better in terms of OS, RFS.
    But, you need to weigh the benefits and drawbacks of chemotherapy to fit with your patient's performance state, energy level, cognitive level and nutritional state.
