Pain in lower abdomen and testes can be due to multiple causes ranging from urinary tract infection, infection of testes, varicocele, or some stone problem in ureter and kidney. You will need a few investigations to see for the cause then only treatment can be advised. You will require sonography of abdomen and testes, microscopic urine test, and renal function test. Depending on the report of this test, we can determine the cause and advice further. If pain is too severe, you can visit a urologist for physical examination.

Investigations to be done

Sonography of abdomen, pelvis and scrotum,with colour Doppler testes, urine microscopic test, and renal function test.

Differential diagnosis

Epididymo Orchitis, urinary tract infection, varicocele,
and intermittent torsion.

Treatment plan

Follow-up with reports.

I think I have some STD. Because I had unprotected sex with someone last year. What all tests should I get done? I had some boils in my head, and they are not getting better. Whenever I take antibiotics, it gets fine and then again, after some days they appear.

The test mentioned above will help us reach a better understanding of the disease. Add blood sugar also in the above test as you have multiple infections. Even if you have STD, it can be treated by medication, but we need to rule out other causes of pain first.

Yes, it can be done. Do western blot or Elisa (enzyme-linked immunosorbent assay) test for HIV. It is more confirmatory.

I have gone through your report (attachments removed for patient’s identity protection). Reports seem fine. There is no human immune virus (HIV) detected in the report. You do not have a urine infection also. Sonography was not done. It could have given us more information about any stone problem causing the pain you had complained of.

You also mentioned boils near the genitalia. It can be due to either infection of hair follicles or infection of sebaceous cysts.

If it is an infection, then neomycin powder will help. It can be applied three to four times a day.

Still, I would suggest showing it to a dermatologist for a physical examination. To be confirmed the diagnosis. Suppose your abdomen and testes pain continues. I advise you to go through sonography of the abdomen and scrotum.

The most common other sexually transmitted diseases are gonorrhea and chlamydia, which present as urethral discharge and pain during urination. Even trichomonas has similar features. Herpes is a very painful blister. I do not think you have any of these features.

Still, in person with the doctor would be the best option for a detailed examination of the genitalia.
