Hello doctor,

A 29 year old female had an abortion through pills. She took Mifepristone initially, and Misoprostol tablets 800 mcg sublingually 26 hours later. She had severe pain and contraction immediately after taking Misoprostol. Two hours later the placenta and blood clots came out. She had heavy bleeding. So, I thought the abortion was over, but two days later, the fetus came out in a rotten stage with a bad smell. Please help. I am worried about an infection.

The dosing schedule was wrong. The basic mistake was you did not know the age of the fetus. This medical abortion is recommended for pregnancies that are less than 63 days. Now, you definitely need an ultrasound of the lower abdomen. It is fortunate that she is not having any fever, bleeding or abdominal pain. I suggest you meet a gynecologist as quickly as possible. She had a fever, but it got cured after taking Misoprostol sublingually. After 3 hours of bleeding and cramps, I.V DNS2 and NS1, Gentamycin half teaspoon, and Paracetamol half teaspoon were given. Now the only complaint is painful and heavy menstruation. My question is, is D and C still necessary as the fetus has come out and the bleeding continues? From the USG, it was confirmed that no residues were present inside. So, can we continue only the antibiotics and IV infusion? Please suggest.

If there is heavy bleeding, she needs a dilation and curettage (D&C) right now. You cannot wait until the USG is done.
