I am a 38-year-old female, and I have acne and oily skin. Recently a pimple on my face started increasing in size and became big as a coin. It is painful too. I can see red as well as slight yellowish color. Kindly help.

I read your query and understand your concern.

As per the history and image, it is acne vulgaris (attachment removed to protect the patient's identity). Acne usually appears due to multiple factors, including hormonal changes, climate changes, dietary imbalances, genetic factors, skin products, and gynecological conditions. For the management of current acne, I would suggest the following:

1. Use Salicylic acid 2 % Glycolic acid foam face wash twice daily, massage gently in a circular motion for one minute, and wash off.

2. Peroclin gel (Benzoyl Peroxide and Clindamycin) 2.5 % to be used in the morning after cleaning with the face wash for two months. Apply only on the acne lesions.

3. Aziderm (Azelaic acid) 10 % cream to be applied in a very thin layer on the affected area on alternate days at night only. Make sure your face is cleansed and dried well. Your hands and face should be absolutely dry without any moisture. Apply the cream in a pea-sized quantity only on the affected areas in a thin layer. Do not use it all over the face; do not use it in large quantities since the medication is strong and may cause irritation.

4. Tablet Ascorbic acid chewable 500 mg twice daily after food for two months.

5. Avoid sweets, oily, fried food, milk, and dairy products.

6. Consume more fruits and vegetables.

7. Drink enough water.

8. Glycolic acid chemical peels done in sessions of at least six every two weeks to 1 month would be helpful to manage acne and recurrent pimples. Intralesional steroids injected into the pimple directly are another method to get rid of them quickly and efficiently. Both these procedures require direct consultation with a dermatologist.
