I have a problem in the sinus with burning. It causes headaches. The nose is very dry and I have pain in my right ear.

You have severe allergic rhinitis with associated sinusitis. Before going to the treatment, I explain the cascade of events in a simple manner. Exposure to dust can initiate the reflex such as sneezing to remove the offending agent out. If this fails, the mucosal cells present in the nose start to produce loads of mucus to trap the dust. This mucus can either be blown out from the nose or gets pushed into the throat initiating a mucociliary activity and it enters a new zone. Initially, the throat reacts by activating the cough impulses to push the mucus out. If this fails, local inflammation is formed around the throat causing symptoms with a feeling of a lump in the throat and there is a repeating feeling to clear the throat. Chronic long-standing inflammation around the nose involves the eustachian tube. It ventilates the air and equalizes the pressure between the nose and ears. These tubes do not function well during inflammation and cause a negative pressure in the ear causing ringing sensation, muffled noise and sometimes, there is a decrease in hearing. Long-standing inflammation blocks the outflow tract of the sinus leading to a collection of secretions. Then it gets infected causing pressure headaches around the eyes, forehead, and cheeks. Your glands also get swollen. Since you have this problem for a long time, I do not whether you have followed any specific treatment. You can follow these measures to treat both allergy and sinusitis. Wear an N95 mask when you go out. Do not allow any dust or irritative substance to enter the nose. You can take tablet Sinarest (Chlorpheniramine maleate with Paracetamol) one tablet two times a day after food for 15 days, tablet Allegra (Fexofenadine) one tablet two times a day after food for 15 days, tablet Zerodol 4 mg (Aceclofenac with Thiocolchicoside) one tablet two times a day after food for five days, and tablet Clarithromycin 500 mg one tablet two times a day after food for 15 days. You can use Duonase nasal spray (Fluticasone propionate with Azelastine) two puffs two times a day in both nostrils for three months. You can take tablet Mucomix 600 mg (Acetylcysteine) one tablet two times a day after food for a week, and take tablet Wysolone 10 mg (Prednisolone) one tablet after breakfast for a week. You can do steam inhalation three to four times a day. You can use salt water gargles eight to ten times a day for a month. Do not smoke or drink alcohol and it slows down the mucociliary clearance mechanisms. Follow these precautionary measures strictly to get maximum relief. After following these measures, your symptoms start to come down and you will recover soon.
