Hi doctor,

I suffer from allergic cough and rhinitis for many years. I am taking Montek for six months. But I do not have any improvement. Do I need any nasal spray? Please suggest some medications. Thank you.

After going through your history and attachment carefully (attachment removed to protect patient identity), I suggest you consult a physician with your reports. Your serum IgE (Immunoglobin E) levels are raised. Allergic rhinitis can occur due to a known or unknown form of allergen. If you are aware of the triggering agent, then you can avoid it. If you do not know the allergen, it can be due to some particles present in the atmosphere such as pollutants present in the air. Try to clean your bed sheets and pillows frequently.

Dry your blanket and mattress under direct sun. Clean your room and house including shelves and accessories using a wet mop or cloth and do not brush them as the dust particles stay in the air. Try to take tablet Levocetirizine with Montelukast twice daily for two weeks and taper off to once daily at night before going to sleep for the next two weeks. You can use Duonase nasal spray (Fluticasone Propionate with Azelastine) two puffs in each nostril by placing the nozzle directed towards the nostril. Do this twice daily for three weeks. Kindly follow these steps and update me after two weeks.
