I have noticed a progressing bald spot on the top of my head. I am a 33-year-old female. Please tell me why this is happening and how I can fix it.

I have viewed the image (attachment removed to protect patient identity). This could either be frontal fibrosing alopecia or androgenetic alopecia or alopecia areata. How long since you have noticed it? Was it gradual in onset or did it appear suddenly as a smooth, bald patch of alopecia?

It was gradual. The attached photos were taken on 21st, 28th, and 13th of last three months. The most recent one was taken on 22nd of this month.

That is a relatively rapid course. I think it is most likely androgenetic alopecia or frontal fibrosing alopecia. Treatment is the same for both. There is thinning of hair in both these types. Frontal fibrosing alopecia usually involves the frontal hairline whereas androgenetic alopecia is more diffuse wherein hair thinning involves the entire top part (front, mid and vertex) of the scalp. I suggest you use Minoxidil 5 % solution, half a teaspoon twice daily. In addition, I suggest you take oral Finasteride 2.5 to 5 mg once a day. Treatment is long term.
