Hi doctor,

I am a 20-year-old female with an IUD (intra-uterine device) for the last four years. I could not feel her strings for the last two months. Then, two weeks later, I started feeling pregnant, and six weeks after that, I finally took a positive pregnancy test. Several days later, I took another test, and it came back negative. The day after taking the test result negative, I started bleeding heavily. Strings of tissue, big blood clots, massive cramps to almost pass out. All the fatigue and I had not bled in four years, not since before my IUD was put in. And when I started to bleed, all the previous pregnancy symptoms were gone—replaced by emptiness and this feeling of loss. I have stopped bleeding, but I am still really tired. Some slight dizziness, lightheadedness, cramps, and sharp pain persists. Kindly help.

I have read your concern in detail and will surely help you out. As you cannot feel the threads now, so we need to confirm the position of the IUD (intra-uterine device). For this one, an ultrasound is required to know where the IUD is and if it has been expelled with the bleeding. Only after that can further management be done. For now, if you have excessive bleeding, then you can take Trenexa 500 mg (Tranexamic acid and Mefenamic acid) thrice daily for three days. This will control the bleeding and, meanwhile, get one ultrasound pelvis done for IUD. I hope this helps.

I have had two other doctors say it was a miscarriage, and getting an ultrasound is extra safe. But I need a straight answer. Judging from what I told you, do you suspect a miscarriage? I have got an appointment that I am booking now. But before I go through the rest of this, I need to know.

Yes, according to the symptoms you have described, it appears that you were pregnant, and because of that, only strings were pulled up into the uterus, and you could not feel them, or maybe the IUD (intra-uterine device) was expelled without your knowledge, and that is why you got pregnant. Getting pregnant with IUD in situ, although possible but is not common. Now, as you had complained of heavy bleeding with passage of clots and pain, it points towards a miscarriage. So this can only be confirmed now after a pelvic examination and ultrasound.
