I have had anal fissures for two days and have developed a fever since yesterday. Kindly help me.

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have had anal fissures, and since the evening, I got a fever. Can fissures cause an infection? If yes, are there any antibiotics I can take? I have a 9-month-old baby whom I am still feeding. Kindly guide me.

Hello,

Welcome to icliniq.com.

Usually, anal fissures do not cause infection. Anal fissures, if present chronically, becomes chronic anal fissures.

Have you measured the body temperature? Is it a mild rise?

Please answer the question for further help.

Patient's Query

Hi doctor,

Thank you.

I have had fissures on and off since delivery last October (it was a vacuum delivery), and the temperature is mild, rising 99.6. Kindly give your opinion.

Hi,

Welcome back to icliniq.com.

Yeah, childbirth or delivery is one of the causes of anal fissures. You have an only mild rise in temperature, and it started today evening, as you mention.

I suggest you take just a tablet of Paracetamol 500 mg for fever at this temperature. Avoid having cold or fridge water intake.

This chat is for 50 hours. If you find body temperature reading more than this in these hours, then I will suggest an antibiotic.

Is it fine for you? Do you have any other symptoms?

I hope this was helpful.

Patient's Query

Hi doctor,

Thank you.

I have no other symptoms, just a fever. I will get back to you if the temperature rises.

Hi,

Welcome back to icliniq.com.

Please get back if you have further doubts.

Patient's Query

Hi doctor,

Thank you.

The fever is still the same, around 99, but the fissures are getting really painful. Is there any chance of infection, and should I start antibiotics? Kindly give your opinion.

Hello,

Welcome back to icliniq.com.

Our normal or mean body temperature is 98.2 F (+ or - 0.7 F). So the temperature of 99 is very mild.

Do you get bleeding during defecation? Is there any swelling at the anal opening? Any pus discharge?

Please answer the questions for further help.

Patient's Query

Hi doctor,

Thank you.

I have no other symptoms. Kindly help me.

Hi,

Welcome back to icliniq.com.

Since you do not have the above symptoms, it could not be infected. And also, the temperature is 99 F which is just above normal body temperature.

Are you bothered because of anal fissure?

I hope this was helpful.

Patient's Query

Hi doctor,

Thank you.

Yes, it is very painful, and it has been four days already. Kindly help me.

Hi,

Welcome back to icliniq.com.

Do you have constipation, passing hard stools? And are you sure it is anal fissure only which was diagnosed early?

Please answer the questions for better help.

Patient's Query

Hi doctor,

Thank you.

Yes, I have had it before, but this time pain is a bit more when passing. I had constipation when it started three days ago, not after that. Kindly help me.

Hi,

Welcome back to icliniq.com.

Apply 2% Lignocaine or Lidocaine gel for pain relief. It is an anesthetic gel that will numb the area and provides relief from pain.

Permanent solution of anal fissure includes:

1) Add fiber (fiber fruits) to your diet.

2) Drink plenty of water.

These will help to prevent constipation.

Avoid straining during bowel movements. These measures will help to heal the fissure. Avoid fatty and spicy foods. Take laxatives if constipation predominates.

The final option is surgery. But usually, doing the above measures, helps to heal the fissure.

I hope this was helpful.

Patient's Query

Hi doctor,

Thank you.

I have a new issue. Today morning I woke up with pain in one side of the throat and ear. I have a history of tonsillitis. Can you please advise treatment for this as well?

Hello,

Welcome back to icliniq.com.

Since it is just one-day history of complaint, better do the following:

1) Lukewarm gargle two times a day.

2) I suggest taking tablet Paracetamol 500 mg once or twice according to the pain severity.

3) If it is associated with a runny nose, take antiallergic medicine.

Wait one or two days for antibiotic intake if symptoms regress by doing above, good or else take an antibiotic. Please do not rush to take antibiotics from the beginning. Let our body's immune system fights off naturally.

Have you applied gel or cream yesterday? How is it now?

I hope this was helpful.

Patient's Query

Hi doctor,

Thank you.

Yes, I applied gel yesterday, and today, the passing stool is still very painful. Kindly help.

Hi,

Welcome back to icliniq.com.

Apply it few minutes before going to the toilet. It is an anesthetic gel that reduces pain temporarily but not the cure. The main aim is stool should be soft, and without abdominal strain, the stool should come out. Cure or complete solution is by doing above measures.

I hope this was helpful.

Patient's Query

Hi doctor,

Thank you.

I will follow the steps.

Hi,

Welcome back to icliniq.com.

Please get back if you have further doubts.
