Hello doctor,

My mother is 58-years-old. She is having severe pain while passing stool for the past two days. The area surrounding the anus is swollen and having redness also. She is having persistent pain, because of which she is not able to sit or lie down properly. The pain increases while passing the stool. She is having a constipation problem for the past several years. She is a heart patient and on the following medicines Calaptin 80 mg, Amifru 40 mg, Acitrom 3 mg, Ocid 20 mg. Kindly help.

It is likely to be an anal fissure. It would be best if you get her examined by a local doctor to confirm it. The cause of it is constipation, and if it is not adequately treated, it hinders the healing of the fissure and may also cause recurrence. At the moment, I would suggest that she take laxatives like Cremaffin Plus (Sodium Picosulfate, Liquid Paraffin, Milk of magnesia) two teaspoons at bedtime to relieve constipation, and the dose may be modified according to the need. She should do a sitz bath for 15 minutes two to three times daily. For pain control, she should use Lidocaine ointment, to be applied locally two to three times a day. Also, she should apply Nifedipine ointment two to four times daily along with her heart medicines. I hope this helps you. Regards.
