Hello doctor,

My 12-year-old son has got anal fissures due to constipation. He has the habit of holding his bowel movements while in public, and we have finally helped him overcome the habit. However, the fissures and spotting have not reduced. He does not have a fever, abdominal pain, blood in stools, or any food triggers that point towards Crohn's disease. His doctor has suggested he get a colonoscopy done. He has normal bowel movements; however, his anus looks red and irritated, and his skin seems thin. Currently, he is using a stool softener. Also, his stool sample does not show blood or bacteria. I have tried using coconut oil and vaseline, but nothing seems to help.

Kindly advise.

I suggest the below treatment plan for five days:

  • Cream Practosedyl BD (Beclometasone, Lidocaine, and Phenylephrine) to be applied to the affected area twice a day.
  • Syrup Cremaffin plus (Liquid Paraffin, Milk of Magnesia, and Sodium Picosulphate) 7.5 ml in 200 ml lukewarm water at night for a few weeks.
  • Tablet Zerodol-SP (Aceclofenac, Paracetamol, and Serratiopeptidase) half tablet twice a day.
  • Tablet Clavum 625 (Amoxicillin and Clavulanic Acid) half tablet twice a day.
  • Tab Pan 40 (Pantoprazole and Domperidone) once daily on an empty stomach.
  • Sitz bath twice a day.

Consult a specialist doctor, talk to him or her, and then start the treatment with their consent.
