Hello doctor,

My mom is immunocompromised, and she took care of a stray cat. This female FIV+ cat had ringworms, pinworms, mites, and something else that cannot be identified yet. Even the humane society we brought the cat to is unable to make her adoptable yet because they cannot cure her of what she has. My mom was diagnosed and treated for pinworms, sarcoptic mange, and ringworm. She still experiences this feeling of crawling all over her body and itchiness near her butthole. She is unable to sleep at night, and she is barely functioning. She has been suffering for two and a half months. I felt itchiness on my butthole for a while, so she had me take the pinworm medication two days ago just in case. She bought me new underwear. One hour after wearing the new underwear, there were these super tiny orangy tan dots when I looked. I put my underwear under the microscope and took pictures of what I see. There appears a small white worm, and the little orangey tan circles look like eggs. Is there any way to send you the pictures so that you can diagnose what I have? I know pinworm eggs are not visible without a microscope. So I do not think it is pinworms. Please help.

I can understand your concern. According to your statement, you have been suffering from the feeling of itchiness on your butthole.

Pinworms are visible to naked eyes. They are about 2-13 mm in size and white. But pinworm eggs are small and transparent and can be seen by a microscope. A grown pinworm is yellowish-white in color. So, the yellowish colored structures may be mature pinworm or pinworm's egg. And pinworm eggs are visible under the microscope.

Treatment of pinworms are Mebendazole, Albendazole, or Pyrantel Pamoate can be taken one dose initially and then another single dose one to two weeks later. Pyrantel Pamoate can be purchased without any prescription.

But I am concerned because I see little dots that are visible to my naked eye. They are not pinworms. And they cannot be the eggs because I can see them without a microscope. Do you happen to know what those could be? Is there another parasite that could present like that?

They may be nits, which are lice eggs. They are usually seen on the surface of an infected person's clothing. As the cat has pinworm, ringworm, mite, so there may be lice too. And lice's eggs are tiny, yellow, white, or brown ovals found on blankets, towels, sheets, or clothing. Washing the affected itchy area with warm water and using Permethrin lotion 1% may help get rid of lice.
