Hello doctor,

I am 34 years old (weight 51 kg, height 165 cm) and have one four-year-old child (conceived naturally). I have been trying to conceive (unsuccessfully) for the last year. My AMH value is very high (10.99 ng/ml). Other hormone values (as measured on the second day of the menstrual cycle) are:

Progesterone 0.2 ug/l.

FSH 6.08 IU/l.

LH 5.20 IU/l.

Prolactin 5.62 ug/l.

TSH 2.54 mIU/L.

Testosterone 0.37 ug/l.

DHEA 4.9 umol/l.

Insulin level (on empty stomach) is less than 2.00 mE/l.

During the IVF treatment, I was given Puregon 100 IU from the 2nd until the 9th day of the cycle and Orgalutran from the 7th until the 10th day. I was given Ovitrel as the trigger shot (9th day). On the 11th day, egg retrieval was performed. I had only one follicle, which was empty.

I would be very grateful if you could advise me on the following two matters:

1. Why is my AMH level elevated (it seems it is not due to insulin resistance)?

2. Are there any specific IVF protocols for elevated AMH (the ovaries seem unresponsive)? My IVF physician suggested Menopur 150 IU.

I read your query and can understand your concern.

The normal level of AMH (anti-mullerian hormone) is between 1.5 to 4 ng/ml. If AMH is low then it means low ovarian reserves but if it is high it shows polycystic ovarian syndrome or hyperstimulation syndrome.

As you have been treated for infertility and have been tracking ovarian follicles for ovulation, fertilization, and then implantation, high AMH ovarian hyperstimulation treatment is needed to reduce the side effects of this syndrome. If you feel whole-body puffiness, abdominal width increase, or other abnormal symptoms it is better to have an immediate hospital visit and have a check and intervention to relieve symptoms as these could be dangerous.

Stay in contact with your IVF (in vitro fertilization) specialist and update me regarding your new symptoms and monitor properly.
