Hi doctor,

I am a 20-year-old male suffering from a kind of health anxiety. Before the pandemic, I was anxious about my health. I had a change in my bowel habits and because of this, I started to pinch and touch my abdominal wall constantly. I palpated some veins under my skin and started examining them. It became a terrible habit in me and it lasted for seven months. I would pinch and pick the veins on my stomach from left to right again and again. I did it due to increased stress and anxiety during the pandemic. Now, my skin had never become red in the area where I pinch. I usually have slight irritation on this area after touching it with bare hands.

I do not have any scars or blood or wounds over it. I do only picking and I do not remember whether the veins were soft before I picking them. I feel harder but not painful. I have slight discomfort and I think that this has happened due to the movements of those veins from their original site. Probably, pinching has caused them to become superficial or made irritated. This discomfort resulted from lifting my arms and due to constant touching and massaging the area. I would like to know whether this behavior is considered self-harm. Is this a form of body-focused repetitive behavior? Please suggest.

The veins in the abdomen cannot be palpated unless there is a severe liver disease or a block in the major vein that brings blood back to the heart. So it has to be ruled that there are engorged veins in the absence of any disease. The next concern is about your repeated behavior. Yes, it can happen especially when the person is under stress or anxiety. Some people resort to plucking hairs, some rotate their fingers and some start tapping their legs. All these happen without the realization of these persons. Sometimes, they become aware of it and also feels to stop it but still, it resists. In your case, it is anxiety that causes repetitive actions. It may not be harmful but it can cause embarrassment. I am not able to comment on the scars present on the abdomen or redness before examining it. Kindly consult the doctor and anxiety is a treatable condition. Once you identify the cause, you can manage it.
