Hello doctor,

My brother aged 52 years suffering from acute anxiety and fear since two weeks with homocysteine level more than 50 umol/L and IgE 122 IU/ml (other blood, urine pararmeters, and hormones are normal). He would like to start Zoloft 50 mg daily for two to three weeks. Is this fine?

Your brother is suffering from acute anxiety and fear for two weeks. Take tablet Alprazolam 1.5 mg once daily for 20 days. Zoloft 5 50 mg daily for two to three weeks is also fine. Homocysteine level of more than 50 umol/L is moderately high.

Causes among them are poor diet, poor lifestyle especially smoking and much coffee and alcohol intake, some prescription drugs, diabetes, rheumatoid arthritis and low thyroid function.

Use Folic acid, vitamin B6, and vitamin B12 are all involved in breaking down homocysteine. IgE 122 IU/ml is high so take Loratidine 10 mg eight hourly for 10 days.

Increased anxiety may be positively associated with increased serum homocysteine level. A healthy, balanced diet containing at least five servings of fruits and vegetables a day will help to decrease homocysteine level. Daily vitamin supplements neede to decrease homocysteine level. Tablet A to Z Gold one tablet OD one month. Acute exersice increases homocysteine level. High plasma total homocysteine level concentration is associated with increased risk of cardiovascular disease. Consumption of unfiltered or filtered coffee increases total homocysteine level concentrations. Vitamin B12, B6 and riboflavin choline will help to reduce homocysteine level.
