Hi doctor,

This afternoon, when I was taking a nap, I had this odd, intense sensation. It felt like a strong buzzing sound was playing in my head, and I could not control my body. I am unsure how long this episode lasted, but I was conscious and woke up right after this happened. I noticed some tremors and an increase in heart rate after I woke up. I have been feeling these buzzing feelings during the daytime, too, when I am not sleeping, but they are way less intense and not to the point where I can not control my body. This time it felt like the buzzing got 100 times worse, and I noticed somebody tremors afterward. Could this be something I should look more into? Or it could be just one of my anxiety spells.

I would like to know some details about your symptoms:

1. You have mentioned that you are on Lyrica (pregabalin). I would like to know for which symptoms or problem you are on Lyrica and since when?

2. The buzzing sound started during waking up time, and then it remained persistent with lesser intensity with increased intensity in the later part of the day. Or was it intermittent like subsiding in between and then reappearing?

3. The body tremors :

(a) In which body part you had those tremors?

(b)These tremors were visible by eyes too? Or only felt inside the body but not visible?

(c) Were they intermittent or persistent?

(d) If intermittent, How long do these body tremors last in one episode?

(e) Did an increase in heart rate coincide with those body tremors?

(f) Were you have any associated impending doom, going to die, or fearfulness at that time?

4. Do you get such a buzzing sound before also (either with low intensity or intermittently)? If yes, on which side (left or right)?

5. Have you had any ear symptoms like decreased hearing, ear discharge, blocked ear, or pain in the ear?

6. Have you had any weakness (decreased power) of any limb (arm, hand, or leg) during that time?

7. Have you had any chest pain or heaviness in the chest or breathing difficulty during that time?

8. Have you had any vertigo associated feeling of rotation of surrounding or feeling of rotation of self?

Other information:

1. How is your sleep routine?

Do you get sleepy soon after laying in bed, or it takes long time?

Once got sleepy, do you have frequent awakenings during sleep?

Do you feel fresh on awakening from sleep in the morning?

2. How is your usual mood in day-to-day activities? Happy or sad side (thinking about past events). Toward the worried side (thinking about future things, irritable, or something else)?

Investigations to be done

The investigation is to be done on T3 T4 TSH (thyroid stimulating hormone). Serum Vitamin B12, MRI (magnetic resonance imaging) brain, including the bilateral inner ear, ECG (electrocardiography) and ENT (ear, nose, and throat) consultation (physically).

Differential diagnosis

Differential diagnosis

Tinnitus.

Thank you for your reply.

About two months ago, I was put on Lyrica for anxiety, 50 milligrams a day. The buzzing sound is intermittent. I experience them randomly throughout the day. They also last between two to five seconds randomly. For my body tremor, I noticed it after the episode ended. It was mostly my upper body. They were not visible. I only felt them inside the body. The body tremor only happened after the episode and never happened again. An increased heart rate was associated with this episode, especially after it ended. I associated the impending doom feeling when it happened. I thought I was having a seizure of some sort and felt I was about to pass out. But after it happened, I woke up from it. The buzzing sound only started about a week ago. It is hard to tell which ear it is coming from. I do not think I have had any ear problems recently. I did have some ear problems when I was young. I remember it was my right ear, but it was a long time ago, and I do not notice any hearing problems these days. I do not remember having chest pain or difficulty breathing during the episode. For vertigo, I think when the episode first happened, I might have had some vertigo, but it is hard to tell cause I was sleeping. My sleep routine is good. I do not get sleepy soon after I woke up. I feel normal after I wake up usually. My mood these days are often associated with worrying about my symptoms.

As per your description, you have been having anxiety for two months, starting with Lyrica. And this time, your symptoms are intermittent buzzing sound inside your head or ears and a feeling of body tremulousness (though not visible by eyes) associated with impending doom and a worried mood.

These symptoms may raise the possibility of severe anxiety or panic attack.

The buzzing sound may happen in psychological issues, and there are many more reasons for buzzing sound (medically called tinnitus). It can also happen in diseases of the ear or some brain conditions.

I would suggest you consult an ENT (ear, nose, and throat) specialist physically and also get an MRI (magnetic resonance imaging) brain along with a few blood work to search or rule out a few things.

Lyrica is not among the first-line medications for anxiety. I suggest you consult a psychiatrist for a detailed psychological assessment and further management of anxiety issues.
