Hi doctor,

I am getting arrhythmia since when I was 15 years old. It is usually brought on by exercise and emotions. Now I am 34 and getting a new symptom in the ECG tracing at rest. Mild to moderate pulmonary valve leaking is seen. Can I get an opinion on the tests attached and my ECG? Kindly give your opinion.

What is your weight? Do you have any history of snoring, interrupted sleep, excessive daytime sleepiness? I went through your reports (attachments removed to protect the patient's identity). In the tracing, the initial and last part shows sinus rhythm, while in the middle part, there is supraventricular rhythm, likely atrial fibrillation with a fast ventricular rate. Get a thyroid profile done. Also, a repeat echocardiogram should be done to see the pulmonary valve status and new development (if not done recently). And you can request antiarrhythmic medications to be taken when such arrhythmia reoccurs. I hope this helps you get back to me if you have any doubts or more information is needed.

Hi doctor,

Thank you. My weight is 198 pounds, and I am very fit. My echocardiogram was fine last week. The ejection fraction is 55%. I wake up sometimes gasping for air and tachycardia and feel fatigued lately. This arrhythmia happens maybe once a week for 20 seconds. I am extremely stressed lately and wondering, does this have an effect on it? And can I continue to exercise normally? Kindly give your opinion.

Your symptoms may be related to arrhythmia. Yes, stress can precipitate this. Addiction like alcohol, smoking, too much coffee, or sleep deprivation can also precipitate this. Avoid heavy activities till this is resolved and later can be resumed. I hope this was helpful.
