Hi doctor,

My husband is having arthroscopic surgery soon. We did not realize until we went to see the nurse in the hospital for a pre-operation interview that he was still taking an Aspirin regimen. They called the doctor and said it was still good for him to get the surgery. However, I am not so sure. He had the hardening of the arteries and had a mild heart attack about eight to ten years ago. He takes Ramipril, Aspirin, and Allopurinol, and I do not remember but something for the arteries and a water pill. Please advise me if it is safe for him to get this surgery. They want to give him a block as well.

Patients on Aspirin are at risk of bleeding during surgical procedures. Arthroscopy is a low-risk procedure, but various arthroscopic procedures are fairly invasive, and bleeding can occur. The same holds true for blocks as large volumes of fluids are injected close to a nerve. If surgery is not an emergent or life-saving nature, Aspirin should be stopped one week before surgery, but short-acting blood thinners should cover this duration if the risks of stopping Aspirin outweigh the bleeding risks.

I will ask you to discuss your apprehension with your surgeon as you have not told me what surgical procedure in arthroscopy is planned. Also, I do not have your husband's complete medical history.
