Hello doctor,

I have been trying for pregnancy for the past six months. Recently I took my SA analysis and attached the report here. Observation shows teratozoospermia. Could you please let me know if I should try for pregnancy or opt for other options like ICI or IVF?

The report (attachments are hidden to protect the patient's identity) shows you have asthenosphere-teratozoospermia and not just teratozoospermia. The counts are fine, and ejaculation is adequate, which indicates that sperms are getting morphologically deformed due to evidence of infection in semen, as evidenced by the presence of WBC (white blood cells). The parameters of preference are for IVF (in-vitro fertilization) standards but not for natural conception. You can be treated medically, provided you have three months to spare. Or else you may opt for an IUI (intrauterine insemination) procedure where post-washed good motile sperm are transferred directly inside the female uterus.

It is asthenozoospermia because your rapidly progressive sperm motility is just 15 % which should be more than 80 %. Any inflammation, heat, warmth, pressure, local injury, or poor hygiene can increase WBC in the seminal fluid, and infections could be just one of the reasons.

The reasons can be multiple, as mentioned before, poor hygiene, increased pressure over genitals, especially while sitting cross-legged, bicycling, riding, or sitting for a longer time, use of possible ayurvedic drugs, the decreased blood supply to genitals, excess warmth in the genital area, wearing of tight undergarments, and a sedentary lifestyle. Your partner's reports are overall fine but show LH/FSH (luteinizing hormone and follicle-stimulating hormone) of more than 1 mIU/mL and BMI (body-mass index) on the higher side, so she has a propensity towards developing PCOD (polycystic ovarian disease).
