Patient's Query

Hello doctor,

I am a 28-year-old female. I have extremely dry skin and it is reacting with almost every product I use. Mostly, it is becoming dark and now, it started getting saggy and I look older to my age. Though I got advice from many dermatologists none helped. One of them said that it is atopy and it will be cured. But even after two years, no use. Another doctor said that I got sun allergy and it will subside soon but it has been a year and no use. Now I am feeling very shy to go in a crowd because of this aged look and I appear old compared to my husband. I have thyroid and on regular medication. It is under control now. My weight is not decreasing. I cannot run because of asthma and cannot do cardio. Please help.

Hello,

Welcome to icliniq.com.

Any history of atopy or allergies? History of photosensitivity, dust exposure, polluted environment, or humidity? Do you have dry patches, scaling, itch, or irritation? You can use Emoderm cream in the daytime for 15 days. Please attach the picture of the lesion for details.

Patient's Query

Thank you doctor,

Yes, I have a history of atopy, that is what my doctor said two years back. I have dust allergy and every morning, I get sneezes and my right hand is a little troublesome i.e. it becomes swollen and rigid. But major problem is water retention in the abdominal area, and face. My face appears very chubby all of a sudden that too when I need to attend a gathering. When I try to apply anything, it appears more dry. 1.5 years back, we went to Andaman and my face became extremely black when compared to those who accompanied us and when I visited a doctor, he told me I have got sun allergy because I have a burning sensation. He gave me some medications like antoxid and a night moisturizer which made my skin more dry and itchy after applying. I do not even visit a beauty parlour. But people asked me to try and when I got a detan there is no use for which my husband has got good results. Then I used biotique detan cream and it worked but made my skin more dry. So I stopped using anything on my skin. It will be clear but suddenly becomes dry and aged look. Seeing this I ordered st. Botanica face wash and moisturizer which are waste of money on my skin. My face got irritated. If this is the case how can I take care of my skin? Nowadays, I am applying only coconut oil. I am not overweight but I appear to be fat because of my face and double chin. I cannot stay in a cool and air conditioned environment nor in too hot climate which is the case at the place we live. In my childhood, I used to have frequent skin infections and all my legs were filled with a typical skin infection which gives me itching and sometimes blood. After using homeopathy, it subsided and now it is good. I can never tell you when and how my skin behaves. I cannot even use makeup either. Please do tell how many days I can use my cream. I do not have any external marks on my skin but it burns a lot. Please explain my skin condition and all the care I can take. We are also planning for kids. Prescribe accordingly.

Hello,

Welcome back to icliniq.com.

My diagnosis is atopic dermatitis with xerosis (dry skin). Moisturize the skin with Cetaphil moisturizing lotion after shower. Use Cetaphil soap for bath and Cetaphil cleanser twice daily to wash face. Use photostable sunscreen 30 minutes before going outdoors for 20 days, Emoderm cream in the daytime for 15 days (emollient), petrolatum jelly at night on lips for 20 days, and Nexglow cream or Kojic cream in the evening for 20 days (on hyperpigmentation area). Take capsule Becosule once daily for 30 days. Avoid coconut oil as it lead to dryness of skin after some time of application. Also avoid frequent bathing, using harsh soaps, perfumes, detergents containing drying agent alcohol, dusty and polluted environment, and occlusive cosmetics. Take sun protection measures and reduce sun exposure. Stay hydrated and drink lots of water. Take diet rich in zinc, vitamin E, and Omega-3 fatty acid. As a home remedy you can use olive oil or almond oil for limbs (not on the face), it soothes the skin. Avoid low humidity, dry winters, prolong use of air conditioners, exposure to heat/sun as these can also lead to dry skin. Take tablet Atarax (Hydroxyzine Dihydrochloride) 10 mg thrice daily for seven days (for itching).
