Hi doctor,

I have never experienced any pimples on my back, but after getting my first-time wax (honey wax) on the first day, I started having rashes, and it is present for over a month, and there are so many pimples on my back they are very itchy. I do not know of my medical condition. I did my full body wax, but only on my back; there are many pimples. It has been over a month now, and they are just spreading in huge amounts.

After going through the history provided by your end, back acne or back folliculitis is caused due to inflammatory response of the skin around the hair follicles caused due to bacterial contamination or infection, most often from triggering of normal commensal bacteria present within one's body. They may also occur due to the presence of bacteria from an external source. Back acne or folliculitis on the back is most commonly caused due to changes in sebum secretion due to excess sweating and stagnation of moisture or sweat, which is difficult to get rid of from the back. Also, they may occur due to hormonal imbalances, dietary changes, climate changes, genetic factors, stress, and using skin care products. PCOS (polycystic ovary syndrome) is also a common cause which is caused due to hormonal imbalances. In case of other symptoms like menstrual irregularities, kindly consult a gynecologist in person to undergo the necessary tests to rule out their possibility.

I suggest:

1. Seborbar soap (Salicylic acid) for washing and cleaning the area. They are used by lathering with water and applied on the affected area only, leave for three minutes and wash off. Use them during batching. They may be continued till the lesions subside.

2. Clinmiskin gel (Clindamycin and Nicotinamide) may be applied twice daily on the affected spots in the morning and on alternate nights. They may be continued for months.

3. Aziderm (Azelaic acid) ten percent cream may be applied on alternate nights alternate to Clinmiskin. They are a strong medication and, therefore, must be used with caution. They may be applied in a very thin layer on the affected spots only in the smallest quantity possible. Apply and massage in well and make sure to wash your fingers following application. They may lead to irritation, redness, and a burning sensation for first-time users, especially if used in large quantities. Therefore use them on alternate days initially, and once your skin gets accustomed to them, they may be used daily at night. In case of skin sensitivity or redness, they may be avoided, and Clinmiskin gel alone may be used instead.

4. Capsule Doxycycline 100 milligrams twice daily to be taken orally, once in the morning after breakfast and once at night after dinner for seven days. This is an antibiotic and, therefore, may be used only if there is no previous history of allergies to antibiotics. In case of allergies, avoid the medication.

5. Tablet Pantop (Pantoprazole) 40 milligrams, 30 minutes before breakfast and dinner, to prevent acidity for seven days.

6. Avoid oily food, fried items, sweets, red meat, milk, and dairy products. Consume more fruits, vegetables, and fiber. Make sure to consume at least 12 glasses of water in a 250-milliliter glass size daily. They help in preventing the formation of acne.

7. Tablet Limcee (ascorbic acid) 500-milligram chewable tablet to be chewed after dinner for one month. During their consumption, extremely hot drinks or food may be avoided.

Having said that, acne may be on and off and may take months to completely subside depending on external and internal factors, as mentioned above; therefore, consistency with the treatment is advised. Kindly stop the antibiotic after seven days of usage.
