Hi doctor,

Yesterday evening, while bending over and bathing my daughter, I tried to stand straight and had extreme pain down the lower half of my spine. At the time of the incident, it felt as if a tight string down my spine had been plucked and caused shooting severe pain from my tailbone up half my back along the spine. The pain continued all night and is worse today. I have minimal movement, and the pain is constant without moving but worse when I try to move. I am unsure if I pulled a muscle or something worse or if I should go to the hospital. I am a 23-years-old female with a current medication (Suboxone Film). Unfortunately, narcotic pain medications are not allowed for me. Please help.

I suggest the following,

  • Avoid lifting heavy weights.
  • Avoid arching your back; bend your knees to pick up something from the floor.
  • Avoid sitting for long hours.
  • Avoid two-wheeler rides.
  • Improve your posture while sitting.
  • Try to get up from your chair every 20 minutes, take a quick walk around, and then continue working.
  • Always sit on a chair with back support.
  • Do not get up suddenly from a sleeping position. Instead, turn to the side and then get up.
  • Take bed rest if severe pain.
  • Do hot fermentations locally, over the affected area, at least twice daily.
  • Apply Oxalgin gel (Diclofenac, Methyl Salicylate, and Menthol) locally twice daily.

It would help if you took the following medicines,

1. Tablet Pregabalin (75 mg) once at night for ten days before food.

2. Tablet Ibuprofen (400 mg) twice for ten days after food.

3. Tablet Rabeprazole (20 mg) twice for ten days before food.

4. Tablet Shelcal (calcium) (500 mg) twice for 30 days before food.

5. Tablet Vitamin D (60 K) once daily for ten days, then once a month and for 11 months.

Kindly consult with your specialist before taking these medications.

  • It is best if you do the following,
  • McKenzie's back extension exercises.
  • Back core strengthening exercises.
  • Glutei strengthening exercises.
  • Passive straight leg raising exercises.

These exercises can be done initially under a physiotherapist's supervision or learned at home. Start physiotherapy once the pain reduces. Only regular exercises can give results.

After the pain reduces, the following activities are good for you,

  • Swimming, yoga, and walking are suitable.
  • Jogging, running, and weight lifting may aggravate back problems.
