Hello doctor,

I am having back pain near the spine on the right side, especially when I go downstairs or go for a walk. My back gets locked when I am standing still. There is too much pain on the right side while sitting on a hard surface. There is also nerve pain after having food sitting on the ground. There is pain only on the right side while using the commode. I am suffering from this pain from the past two years and have not got any relief. Kindly help me.

I have gone through your reports (attachments removed to protect patient identity). I would like to know a few more details regarding the characteristics of pain and previous history such as, what is the type of pain that you are having? Is it an electric shock like pain or dragging or aching pain? Is the pain extending to the right leg or is it only on the back? If extending to the leg, then up to what level do you have pain? Do you have tingling sensation in right leg or weakness? In these two years, has the pain increased or decreased or is it continuing same? What is your occupation? Do you have any past history of injury or fall or any other disease or pain in any other joint in the body?

    I have a feeling of continuous pain and the pain is not electric type. The pain does not extend to the leg, but there is pain in the right hip when I stretch back. The pain is not too hard to bear. The intensity of pain is about level three out of five. I do not have any tingling sensation and weakness in any leg. There has not been much increase in pain. I am a software development professional, but I do not sit more than half an hour continuously. I do take breaks after half hour of sitting. I had no injuries previously. I am also having pain in heels and ankles and so, I cannot stand for more time. I cannot sit without support because of pain. Mostly I have to sit straight with support because when I bend, there is a pain in the spine. It looks like my back joints are added to each other. There is pain in the place where the spine ends on the right side. There is also pain while bending spine in front. Let me know if you notice any problem in the MRI.

    It looks like there is a problem in your sacroiliac joint on the right side. It is not due to nerve compression. The exact diagnosis would be possible only after your clinical examination. The MRI images are not very clear. However, there is no problem that is evident on the MRI in relation to your complaints. So, it looks like a sacroiliac joint arthritis. You need to start exercising for the same and for this you will require a physiotherapy consultation. By that time you can start on a short course of painkillers. Even after adequate physiotherapy for about a month, if there is no improvement, then you might need to take local injection therapy for pain. Do not lift weight and do not bend your back. Also, avoid prolonged sitting.
