Hello doctor,

I am a 28-year-old male, and I have been experiencing lower back pain only on the right side for the past two years. I was diagnosed with tuberculosis and I recovered within 12 months, but the pain still persists. I have done a CT scan, and MRI spine, but the results are normal. The orthopedic and urologist concluded that there is no issue. I also visited a physiotherapist and followed the exercises for at least a month, but no improvement. No doctor is able to identify the root cause of this issue. They only say that it is a muscular pain so ignore it.

Thank you for your query. I understand your concern. You have mentioned pain in your lower back region for the past two years. I reviewed your reports (attachment removed to protect the patient's identity) initially diagnosed as a case of tuberculosis with D8 vertebral involvement as it is written on your MRI report. Also, adequate treatment for Pott's spine has been done with around one and a half years of ATT (anti-tuberculosis treatment). Now please tell me where exactly are you feeling pain, because your initial involvement was around D8 vertebral level which is not exactly the lower back as has been mentioned by you right now. I have a few questions for you.

  1. Do you have any associated pain which goes to your legs, any associated numbness, weakness in your legs, or any abnormal electric shock-like sensations which we call paresthesias, etc?
  2. What is the intensity of your back pain say on a scale of 1 to 10?
  3. How much distance can you walk on an everyday basis and what are your analgesic requirements?
  4. Also as of now do you also have any constitutional symptoms like fever, weight loss, night sweats, etc?
  5. Also to evaluate we have to understand whether your pain currently is lower back which we call a lumbar spine or mid back or dorsal spine region. Get the following investigations done.
  6. Screening MRI (magnetic resonance imaging) of your whole spine with a dedicated study of the involved area.
  7. X-ray of dorsal and lumbar spine (lateral flexion and extension views).
  8. CBC (complete blood count), ESR (erythrocyte sedimentation rate), and CRP (c-reactive protein) reports done recently.
  9. Also kindly send me a clinical pic of the affected area of the back region, so I can understand whether you are talking about your dorsal or lumbosacral spine.
  10. Right now you can take anti-inflammatory medications as per your body's suitability but I can recommend you in detail only once I have all details in front of me.
