Hello doctor,

I have had lower back and hip pain for the past 10 years. Is there a treatment without surgery? I have tried physiotherapy and chirotherapy.

Thank you for your query.

I understand your concern. If there is any pain on pressure at the site and pain in the buttocks so it is more likely to be SI joint problems. Also, we can confirm this by doing an X-ray of SI joints and lower spine, AP view. If needed MRI could be done. Is this pain associated with other joint pain, like wrists, knees, etc, or any stiffness in the spine? If it comes out to be SI joint pathology then treatment is mainly medicine. We need to know the exact cause for the same.

Also, you need to get ANA, vitamin D, and calcium levels checked.

There is medical treatment is available for the same and we can start after the reports. Till then regular exercises, hot water fomentation, and over-the-counter medicine if needed.
