Hello doctor,

I have balanitis for the past 11 months. I have taken two tablets of Fluconazole with daily application of Candid Clotrimazole cream. Now I still have slight redness in the glans penis and sensitivity when I wear tight formal clothing at work. Any idea how long will this take to resolve?

Balanitis can take a little more time to resolve. Apply moisturizer to the glans and prepuce. Do you have prepuce? Or are you circumcised? If you can upload the photo of the diseased part, it would have been convenient for me to diagnose.

I have seen the picture (attachment removed to protect patient identity). Retract the foreskin completely. Wash with soap and water and maintain hygiene. There are a lot of smegma inside, wash and remove it. Apply moisturizer on the glans and prepuce. Take tablet Azithromycin (Azithral) 500 mg once daily for five days. Wait for another week to go. If it does not resolve until then, please contact your local urologist.

You wait for one week with antibiotics treatment. If it does not resolve, you can undergo circumcision. The indication is recurrent balanoposthitis. It is not an emergency. But it is preferable to do it as early as possible.

Are there any negative effects of circumcision? Is it a procedure that needs more days of hospitalization? Also, is it a painful procedure that is done on general or local anesthesia?

Circumcision is a daycare procedure. That means you will be discharged on the same day of operation. It is done under local anesthesia usually. But if you feel nervous, it can also be done under general or spinal anesthesia. It is not that painful. In the postoperative period, you will be prescribed analgesics. So there will be no pain or minimal pain. Do not worry. It is a common procedure with a very less chance of complications. There is no negative effect of circumcision.
