Hello doctor,

I am a 20-year-old male, and I am sexually inactive. I have bright red patches on the penis glans and foreskin for many years. I used tablet Augmentin 625 for seven days, but it did not go away completely. It sometimes itches but not much. A little white fluid comes near the red patch. Can you please tell me the solution?

Is your penile hygiene improper? Are the lesions itchy? Are there recurrent episodes of balanitis? Is there any inflammation and swelling of the glans? Do you have any history of fungal infection in the past? Do you excessively scratch and rub the lesion? Is there any rough handling? Do you masturbate? What is the frequency of masturbation? I have not circumcised. I usually maintain proper penile hygiene; I even retract foreskin while bathing. Itching is minimal sometimes. It has been one to two years since this red patch or lesion appeared, but there is no swelling. For as long as I remember, I never had any fungal infection. I masturbate three to four times a month.

It is balanitis with a secondary bacterial infection. I suggest you with the following medications. Use Candid cream (Clotrimazole cream 1% w/w) at night for seven days. Use Hydrocortisone 1 % gel at daytime for 10 days. Use KZ soap (Ketoconazole) for bathing for two months. Maintain proper genital hygiene. Use Mild cleansers like Cetaphil or Nosoap cleanser, or Episoft cleanser for 10 days. Apply the cream with clean hands. Please avoid scratching or rubbing lesions as it may cause secondary bacterial infection. Kindly answer the below questions for confirmation of diagnosis. Do you have a history of zip fastener injury? Do you use harsh soaps? Did you have any chemical contact? Is there any irritation around the area? Are you prediabetic? Is there burning micturition or an increase in the frequency of urination? Is there any tingling sensation?

Hi doctor,

Thank you for your help. I use Dettol soap; other than that, I have had no chemical contacts. I have never tested blood glucose, but I am not prediabetic or diabetic. Zip fastener injury happened a couple of times in childhood, not now. There is no problem with urination. As it is a secondary bacterial infection, should I use any antibiotics? Is there anything I should be concerned about?

Avoid harsh soap like Dettol as it may cause burning or irritation, worsening the lesions. Get Fasting and Postprandial blood sugar levels checked. Take Tablet Zifi (Cefixime) 200 mg twice daily after food for seven days. Apply T-bact cream 2% (Mupirocin 2% w/w) after Candid cream for six days. There is nothing to worry about. You will be fine in 10 days if you take proper treatment and follow instructions as mentioned.
