Hi doctor,

I am 85 years old. I have dizziness, vomiting, and positional vertigo for the past 22 days, and I cannot undergo exercise. I tried with Vertin, and there are no results; later on, the doctor gave Stugiron. Also, have high blood pressure and currently under medication. Vertigo is mainly positional and severe. Please suggest some remedy.

With the symptoms you have, there is a possibility of a BPPV (benign paroxysmal positional vertigo). In this condition, there is the displacement of the calcium carbonate crystals present in the inner ear, causing hyperfiring signals to your brain and thereby having a feeling of dizziness or imbalance when you look towards the triggering posture. The most common cause attributed to this condition is vitamin D deficiency, and hence I would first advise you to get your vitamin D levels checked. I would advise you to shift over to Meclizine 25 mg twice a day after food for the first ten days. If present, BPPV can only be corrected by a positioning maneuver (called the Epley's maneuver), which replaces crystals into its original place in the inner ear. However, make sure at the same time you do not expose yourself to the triggering factor out there (in your case, lying down to the triggering positional side). Please stop both Vertin (Betahistine) and Cinnarizine as it does not treat the cause but temporarily reduces the symptoms. Please do consider a good multivitamin as there could be other vitamin deficiencies, which could be minorly contributing to the concern. If your vitamin D levels are low, it requires appropriate correction, and with time things should improve. An ENT specialist should do Epley's maneuver, and please do not try doing it on your own as improper maneuver can further increase existing vertigo. I hope this helps you.
