Hello doctor,

I was recently diagnosed with bipolar disorder, depression, anxiety panic disorder, PTSD depression 1 and 2, manic and hypomanic. I am also on Latuda 20 mg. I recently ran out of my prescription, and my doctor refuses to give me any sample packs because I went over the limit. My bipolar disorder is the worst. It keeps me up at night, and sometimes I have really bad nightmares because of which I do not want to sleep. Latuda 20 mg helps but without it, I am just helpless. I have not slept in the last 2 to 3 days because I am scared that I might not wake up. They are that bad because of the trauma I had in the past and the car accidents. I am tired of suffering this all. Sometimes the impact of the panic anxiety attacks gets so bad that I cannot breathe, and it feels like an elephant sitting on my chest. I just feel like the doctor I see now is not really doing much about it. My depression sometimes makes me want to stay in bed all day and I would not even take my dogs out. I would not even leave the house but otherwise I am a very active sportsperson. I hate who I have become. Please help.

I can understand your situation.

You may have Major depressive disorder (as per the above details). There is non-pharmacological management for it, you should try to follow the below guidelines to deal with it. It will help you.

1. You should exercise daily for 30 to 45 minutes like at least going for a walk. It will uplift your mood. You must go out of the home. You have to push yourself to come out of it.

2. You should do meditation for 20 minutes daily. Just sit in one place in a comfortable position and try to concentrate on your normal breathing. Do not stop or force yourself to stop your thoughts, just let them be. It will reduce your anxiety.

3. You should eat healthy home-cooked foods, and avoid junk foods.

4. You must drink enough water.

5. You should spend quality time with friends and family daily, or do video calls if you are living away from your family.

6. You should spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills, language, or musical instruments.

7. You should take 7 to 8 hours of sleep daily.

8. You must avoid tobacco, alcohol, or any substance abuse.

9. Live your life, do not just spend it. Be bold and confident.

You must continue your above medication. You can try taking the tablet Escitalopram 10 mg daily in the morning. However, please consult a physician, discuss with them, and start taking the medications with their consent. I am sorry to say that I can not prescribe you medication. However, you must do the following blood reports.
