Hi doctor,

I am a 26-year-old female taking medicine for bipolar disorder, namely Oxetol 450 twice, Ciplar LA 40 twice as I have a high pulse, Qutipin SR 300 once, Librium 10 at night, Trazonil 100 once, Baclof OD 30 as I had alcohol use disorder. Despite taking these medicines, I have symptoms of restlessness, racing heart, lack of consistency and concentration, and impulsivity. Do I have bipolar disorder? I have a bad habit of changing doctors frequently, but this time I try to stick to one doctor; I do not know what is happening to me. My hands are trembling while writing all this. Please help.

I understand your concern. We cannot diagnose the bipolar disorder based on MCMI (Millon clinical multiaxial inventory) or any other personality test. You have to provide me with more details about your symptoms to diagnose bipolar disorder. When did you first notice the symptoms? What were your presenting symptoms? How did they develop, and how did they progress? The medicines you are taking are mood stabilizers used in bipolar affective disorder. The restlessness you are seeing could be due to medicines or could be due to symptoms of bipolar or any other issue like anxiety. Risk-taking behavior, over-talkativeness, disinhibition, increased anger, big ideas, big claims, increased sexuality, decreased need for sleep, etc., are common symptoms of bipolar disorders, especially in mania. At the same time, the depressive episode presents with different symptoms.

Hi doctor,

I started noticing symptoms of depression a few years ago after having a baby by c-section. Later it worsened into anger, substance abuse, and violence. I have changed ten doctors since. This year I beat my husband as I had developed Benzodiazepine abuse and was not given pills. These episodes of violence happened several times. Nothing helped me. I also quit my job and later tried to revoke the application. I also abused Zolpidem. The present medicines mentioned helped me overcome that. But I am troubled by my inability to make a decision, frequently changing plans and decisions, inconsistency, suicidal thoughts, restlessness, and fear.

I understand your concern. Yes, the symptoms you have mentioned point toward bipolar disorder. Abrupt decisions without thinking of consequences, like quitting a job, beating someone in anger, substance abuse, binge alcohol intake, indecisiveness, restlessness, fear, etc., are seen in bipolar disorder. This could be a mixed episode or a depressive episode. The drugs you are taking, like Oxetol (Oxcarbazepine) 450 twice a day, Quetiapine 300 mg at night, and Trazonil (Trazodone) 100 mg at night, are the right medicines for the symptoms you had. If you are not planning for another baby in the coming months and are not lactating, Lithium can be added to your prescription if your doctor agrees. Lithium is a mood stabilizer and will help in beating suicidal thoughts too, mood swings, inconsistency in decisions, etc., symptoms.

Ciplar (Propranolol) has been added for restlessness and palpitations. But since you have mentioned that you have restlessness, poor concentration, racing heart rate, etc., I think it is best to add some Benzodiazepine like Etizolam should be added to your prescription. You have a history of Benzodiazepine abuse, so the user should be under supervision only. A combination of Etizolam and Propranolol can help you. You can talk with your doctor about that. Another option is to use an anti-anxiety drug like Flupentixole and Melitracen combination. The combination shall help with anxiety symptoms you have and is not having abuse potential.
