Hello doctor,

I am a 18-year-old male addicted to porn for the past few weeks. I would masturbate one to four times a day for the past two years. I feel that my breasts and nipples are enlarging, and there are many marks on my nipples. Also, there are black irregular marks on my penis with rare itching and irritation over the past year. I have attached the photos. Is it gynecomastia or melanoma, or cancer? I have reduced my porn addiction and stopped masturbating excessively.

From the image attached (attachment removed to protect the patient's identity), it does not look like cancer. Instead, the enlargement of the breast and nipple are due to gynecomastia. The hyperpigmented black spots on the penis are due to a superficial fungal infection called tinea versicolor. I suggest you the following. Apply topical Azelaic 20 % cream over the penis lesions in the morning for 20 days. Apply Sertaconazole 2 % cream over the penis lesion at night for three weeks. Avoid oil application over the penis. Do hormonal investigations, culture tests, and USG (ultrasound sonography) tests. If the conditions do not improve, do a skin biopsy. Review after 20 days.
