I have seen the photographs (attachment removed to protect patient identity). These are known as melanotic macules and common on genitals. There is nothing to worry about yet. They are harmless as such and do not require any treatment. If one still wants treatment for cosmetic purposes, laser or radiofrequency can be done. If they change their color or texture suddenly, which is rare, you need to consult a dermatologist at that time, and it might require biopsy. But that is very rare.

No, I will not recommend applying all these things, as it might lead to an irritant reaction. For laser, you need to visit a dermatologist who has a laser center. Laser requires multiple sessions to remove it, once every three to four weeks. If you decide to leave it as such, it is harmless.

Creams are not effective for these. Q-switched lasers are used for hyperpigmentation, including Q-switched Nd:YAG (Neodymium-doped Yttrium Aluminum Garnet), Q-switched Ruby, Q-switched Alexandrite, and more. Anesthesia is required, but in the form of a cream. The cream is applied for 40 to 60 minutes, which will lead to local anesthesia, or the doctor might prefer injections at the site of lesions. The laser itself will then take 10 to 15 minutes. There might be slight redness and crusting after the laser treatment. It needs to be repeated after three to four weeks and might require multiple sessions. Without anesthesia, it will be a bit painful.

Initial redness, and in 10 percent of cases, there might be light hyperpigmentation or hypopigmentation; scarring is rare. You can leave it as it is, as there is no harm. If it changes color suddenly, or there is a sudden increase in thickness or size, you can consult.

Topicals (creams) do not help much, as I explained earlier. Laser is the best option. Nevertheless, you can try this cream once at night, applying a very small amount and avoiding excessive rubbing. Mild redness and dryness may occur, which is normal with these medications. Supatret (a combination of Tretinoin and Clindamycin) 0.04 percent gel should be used once at night. Use it for four to six weeks to see if there are any results.

Actually, there are three to four big (not so big) and several other small mole spots. Should they all be removed using laser? I have had them for around four to five years. Now, I have to remove them. Does laser treatment work in this case?
