Patient's Query

Hello doctor,

I have little black dots on my tongue. They have been there my entire life, and I am still not sure why I have them.

Hello,

Welcome to icliniq.com.

I went through your query, and I am very sorry for the inconvenience caused.

Kindly mention,

  • Do you have any burning sensation felt upon eating spicy or normal food?
  • Do you have any history of smoking?

Keratinization of fungiform papilla noted (attachment removed to protect patient identity) over the tongue with no other visible lesion. Tongue looks clean, and no fungal infection is noted.

Kindly answer the query so I can diagnose the issue and guide accordingly. I will be waiting for your reply. Kindly let me know as many details as possible. I will help you in every possible way, and no need to worry about in any kind.

Thanks a lot.

Probable diagnosis

Suspected pigmented fungiform papillae of the tongue (PFPT), but that would be confirmed only by the reply to the questioned asked. So kindly reply so I can confirm the diagnosis. Nothing to worry and no treatment is needed if it is the case I am suspecting.

Patient's Query

Hi doctor,

Thank you so much for your reply.

There is no burning sensation with regular food, but while having spicy food, which I believe like why I cannot tolerate it, same with mouthwash with alcohol. However, I think everyone experiences it.

Hello,

Welcome back to icliniq.com.

As per your history, I am confident about the diagnosis. Nothing to worry about in any kind. The pigmentation over the tongue is generally seen in a patient with a darker complexion. So, it is not going to harm you, and it is not a concerning issue. Well, most people without such stains also feel discomfort using alcohol-containing mouthwash. So if you intended to use one, kindly choose alcohol-free mouthwash.

1. Capsule B complex (Multivitamin) one capsule once daily after meal for 15 days.

2. Rexidin-M Forte Gel (Chlorhexidine, Metronidazole, and Lignocaine) Or Orajel (Benzocaine) apply over the tongue if any burning sensation.

3. Brush twice gently and use an alcohol-free mouthwash.

4. Use a tongue scraper whenever you brush.

5. Avoid spicy food as much as possible.

You can start the Multivitamin, but I would like to be very specific, no medication can remove the tongue's pigmentation. It will not harm you in any way, and your tongue is normal. You have no restriction over the choice of food or drinks. Please do not try any home remedy or any medication claiming removal of pigmentation over the tongue; it would be dangerous and useless. Please do not worry about any kind. The burning sensation you told about is normal. You are fine, nothing to worry about. Let me know if any further query. I am happy to help in every possible way.

Probable diagnosis

Pigmentation of fungiform papillae of tongue.

Treatment plan

No treatment is needed. The black dots (pigmented fungiform papillae) will remain as they are, and nothing to worry about.

Same symptoms don't mean you have the same problem. Consult a doctor now!
