Hello doctor,

During my recent flight journey, because of privacy issue, I did not go for the toilet and I controlled it for five hours. Next day, I started to feel burning while passing stools. Now every time I feel pain, I can see blood while wiping with a tissue. How to cure this while staying home? Is it possible?

Most of the causes of such rectal bleeding are not life-threatening, but before proceeding with the treatment, the exact cause should be investigated first.

Can you describe the color of the blood? Was it dark red or blackish or normal blood color? Did you also feel nausea or vomiting? Which type of diet are you having? Is there any change from your normal diet? Any history of abdominal pain, loose motion, or constipation history?

Thank you doctor,

Color of blood is red, but not too much blood only at the time of wiping. I do not feel vomiting or nausea. Yes, my diet is changed because two days before, I came to a foreign country and only had dinner and breakfast here. It is a hostel-type food. No history of abdominal pain, and while I was in my country, I felt constipated because I was not doing any physical activity because of the lockdown but it was normal sometimes I felt like my stomach was not as clear as it was supposed to be. Otherwise, there is no background history. Only, I felt discomfort because I was holding it during the journey.

You are having lower GI (gastrointestinal) bleed. My most probable diagnosis would be anal fissure of very mild degree. Since you have a history of constipation and there was burning sensation as you have described previously. It would be due to lack of fibre in your diet. More severe degree of fissure would require surgical correction, but that can be confirmed on only by physical examination. For now, I am going to prescribe some medications. If this relieves your symptoms then there is nothing to worry about.

Increase the content of fibres in the diet. (eg. green leafy vegetables, cereals, pulses). Take Docusate sodium capsule 100 mg take one capsule before going to bed with glass full of water. Do sitz bath (sit in warm water for 5-10 minutes in way that your perineal area remains in contact with it). Also, have plenty of water (three to four liters per day). Do this for three days and let me know in the followup.

I am sure you will feel better.

Docusate sodium capsule 100 mg. Sitz bath. Increase the fiber.
