Patient's Query

Hello doctor,

Today I will discuss the reports of my wife:-

A. Seven days ago, she had a blood test and found the following abnormal readings:-

1. FBS- 120 (70-100)

2. HbA1c- 7.46 %

3.. ESR- 35 mm/hr.

B. Then two days ago, she had a blood sugar test with digital glucometers (without taking her regular medicine) and the following are the readings.

1. FBS- 82

2. PP (after 2 hours)- 190.

C. Then again two days ago, she had blood parameters from another lab and their readings are the following-

1.FBS- 101.79 (70-110)

2. HbA1c- 7.6 %

3. Mean Plasma Glucose- 171.42

D. Yesterday, she had her blood sugar tested in a digital glucometer (with her regular medicine) and found the following readings-

1. FBS- 100

2. PP- 103.00 (after taking her regular diabetic medicines).

Her BP is fine for which she takes S. Amcard 2.5 mg one tablet once daily.

Her concern is:-

A. About sugar, of course, she was a little bit careless and also took sweets due to festivals. She is taking the following medicines-

Nobegliz M- XR (Gliclazid 60 mg and Metformin 500 mg)- one tablet after breakfast.

B. She did mammography, which suggested a few axillary lymph nodes are seen in the right breast and needed USG correlation. A USG of the breast was done four days ago, in which they found the following impression:-

1. Normal fibrofatty changes.

2. Hypoechoic thickening of the costochondral junction with probe tenderness and costochondritis.

Please advise step by step to understand in a better way.

For your wife, fasting glucose is fine. Postprandial glucose is slightly high and HbA1c is not on target yet. She should continue tablet Nobegliz-M XR (Gliclazide and Metformin) daily. I suggest you add a tablet of Istavel (Sitagliptin) 100 mg daily after food in the morning, The USG findings are not of any concern. If she has any pain in the chest area, she can take a simple painkiller like Ibuprofen. Please follow a healthy diet and exercise. Repeat HbA1c after three months. Continue glucometer readings once a week.
