Hi doctor,

What is the best course of action for increasingly concerning blood clots from my vagina? I am a 23-year-old female and they started over a year and have been getting worse. At first, they were red and small but now they are getting bigger. The doctors have dismissed me saying that it is due to my weight but they are getting worse even after losing the weight. Please help.

I have read your query and understand your concern.

For normal bleeding, you will need normal hormonal support to the endometrium. But, based on your history I have noted that your BMI (body mass index) is very high. Your expected weight is 52 to 56 kg. The extra fat in the body is converted to hormones in the body which alters the normal hormonal balance, leading to the poor formation and improper bleeding, which can appear as a brown discharge, fleshy mass, and mucoid discharge and can be treated only by weight reduction. Another possibility is an infection around the uterus. If you have any yellowish, white or grayish discharge, with or without itching or foul smell (especially after sex and periods), pain in the lower abdomen before periods, or feverish feeling, this could be an infection in the inner lining. So, the bleeding will be lesser in amount with fleshy masses, brownish and painful. This needs an evaluation by a gynecologist and a good course of antibiotics and antifungal medicines for you and your partner. Another possibility is PCOS (polycystic ovarian syndrome) for which you will need an ultrasound and hormonal level report.
