Hi doctor,

I have been dealing with this for years and have been too scared to talk about it. About four years ago, I was at a friend's house and noticed my left eye started to go fuzzy, followed by brain fog. I could not remember anyone's name. Then I had a tingling sensation in my left arm. After a while, all the symptoms faded, but the headache continued until the next day. It has been about three years since I have had one of these. A lot of things I have read point toward migraine. What is your opinion?

What is the character of the headache you experience? Is it causing heaviness, stretching, bursting, hammering, or any other type? Please explain. What is the duration of the headache? Do you experience nausea or vomiting when you have a headache? Is there any associated eye redness or watering during the headache? Could you identify any triggers for such episodes? If so, please let me know. Have you had neuroimaging for these headaches, such as a CT (computed tomography) scan or an MRI (Magnetic resonance imaging)? If so, please attach the report.

Hi doctor,

Thank you for the reply.

The episode lasted about 45 minutes, and my headache lasted until the next day. My headache will remain on my brow. I have had about three in the last four years, but none in the last two. They seemed to get better every time I had one. The episodes appear to be getting shorter. I have never experienced nausea or vomiting. There is also no redness in the eye. In the past, I have never undergone a CT scan for this condition. I am constantly aware that I have them. The brain fog only lasts about 5 minutes.

You have not answered all my questions about your clinical symptoms.

According to your description, it could be a migraine headache, and the fuzzy feeling could be an aura phase before a migraine. I would recommend having neuroimaging done to look for or rule out other organic etiologies. And the frequency of headaches is lower if it is a migraine headache with aura (only three times in 4 years). To relieve the headache, you can take any analgesics. Take tablet Naproxen (Aleve) 500 mg in case of emergency at the time of headache. Consult an Ophthalmologist (a specialist who diagnoses and treats all eye diseases and conditions) for refraction, IOP (Intraocular pressure), and fundus.

Investigations to be done

MRI (Magnetic resonance imaging) of brain including orbits, Vitamin B12, Vitamin D3 triiodothyronine (T3), thyroxine (T4), thyroid-stimulating hormones (TSH), and ECG (electrocardiogram).

Probable diagnosis

Migraine with aura.

Treatment plan

Take tablet Naproxen (Aleve) 500 mg in case of emergency at the time of headache. Consult an Ophthalmologist for refraction, IOP (Intraocular pressure), and fundus.

Preventive measures

Maintain regularity in sleep or getting up early and your meal timings. Avoid trigger factors (if perceived any). A 20 to 30-minute jog or exercise session in the morning should be done every day.

Regarding follow up

Revert with suggested investigations.
