Hello doctor,

I am a 24-year-old female. My partner and I used a condom while having sex. But while removing it, the condom came off his penis, and semen spilled over my vagina (outside). Since we were worried about this, I took an I-pill in the morning approximately 12 hours after intercourse. Could I still get pregnant? Also, how should I keep a check on myself to ensure?

For getting pregnant, the sperms have to be ejaculated inside the vagina of the female. The sperm then need to fertilize an egg (available in the ovulation cycle). The fertilized egg then needs to be implanted in the uterus of the female. When all this happens successfully, then the female is said to be pregnant.

In your case, since the condom came out and in the worst case, even if few sperms could have entered inside the vagina, you have taken an emergency contraceptive pill, so there is no chance of pregnancy.

If you can share the details of your last menstrual period, then we can calculate the ovulation and get a fair idea if ovaries would have released the egg. However, in any case, pregnancy will not be there.

I was super worried for the past few days. Also, I forgot to tell you that I have been suffering from PCOS for some years. For the last two months, my menstrual cycles were medium flow for almost five days the previous month and less flow for four days this month. I had intercourse ten days after my periods. Also, is there a need for me to get tested (urine pregnancy test)? This would be a little difficult for me, though. And how should I keep a watch on myself to be utmost sure?

With the details presented by you now, it is evident that you had no periods last month, which may be due to PCOS (polycystic ovary syndrome). And also, you intercourse ten days after menstruation, so there is less probability of eggs being present. Add to this that semen spilled over the vagina and not inside it. So the chances of pregnancy are very rare.

Also, since it has been only four days post intercourse, it will be difficult to detect pregnancy by testing so soon. Do not worry. 99.9% of you will not be pregnant. We may consider testing for pregnancy after a week through a urine pregnancy test once you experience signs like nausea, breast tenderness, etc.

Yes, you may experience breast tenderness, nausea, sometimes slight spotting, or mild abdominal pain because of I-pill. If you miss your periods, please take a urine pregnancy test.
