Hi doctor,

My wife has delivered twin baby girls. She had delivery through C-section. She had normal breastfeeding after 11 days. After that, she got fever with high temperature. So, she went to a hospital. They gave an injection and a tablet for three days. Her fever was gone. But after that, she cannot get milk for feeding. What is the problem?

Delivery is more stressful. There is a lot of physical work. During pregnancy, the requirement for food and blood is double. The chances of anemia, weakness during delivery and after delivery is more. Milk production also depends on many factors like genetically few mothers can produce less milk without any health issue. Illness or medications can reduce the milk production. They can have weakness and poor nutrition. They can also have less milk intake and multiple pregnancies too which can reduce the milk production. It depends because few ladies do breastfeeding till three months or more and they will use formula milk whenever needed as per their convenience as well.

Better feed is the breastfeed actually. Was fever associated with an infection? What medications were given to her? Any other health issue? Taking foods and multivitamins has to be doubled as she had a twin pregnancy. There may be weakness due to more blood loss during delivery. It can also reduce the milk production. Have a healthy diet, with multivitamins, plenty of fruits, vegetables, milk can also help to increase the milk production. Some drugs can also increase the milk production as certain drugs increase the action of prolactin hormone, which can increase the milk production. I think illness, medications, and weakness are the reasons for the reduced production of milk. It is better to have a natural increase in milk production by food, health, and better positioning of babies for sucking. As she has two babies, she can better go for formula milk along with breastfeeding to fulfill the babies requirement.
