Hello doctor,

I had unprotected sex a while back and took a contraceptive pill on the fourth day. I was worried about getting pregnant and expecting my periods today. I do not have any period cramps that usually come by this time, and the main thing is I have this thick bloody discharge. Initially, the discharge was brownish, but it has turned red and is very thick. I experienced some cramps in the lower abdomen along with the discharge. I feel my stomach bloating. I feel hungry after taking I-pill. Kindly tell me what I should do next. I am worried about getting pregnant.

As you have taken I-pill (Levonorgestrel) in this cycle, irregular bleeding and discharge can occur. So first, we need to rule out pregnancy for further management. I suggest you take a urine pregnancy test and revert with the results.

I have reviewed the attached images (attachment removed to protect the patient's identity), and there is nothing to worry about. Sometimes this type of dark bleeding is common before normal periods, which usually happens due to hormonal disturbance in the body. Therefore, it will be fine after one or two cycles. However, you can purchase a pregnancy kit from a nearby pharmacy to rule out pregnancy. The kit consists of a dropper with the help of which you can take two drops of an early morning urine sample and put it in the space provided in the kit. A single line signifies a negative result, and two lines indicate positive results.

Have you taken the test? I have advised you twice to take the test. Nobody can guarantee you regarding anything. Take this test and if this is negative there are other methods to rule out pregnancy. I have already told you the reason for delayed periods that you have taken I-pill (Levonorgestrel) and after this pill, it is very common to have irregular cycles

Due to some work and personal issues I could not go out and as of now my bleeding has started but it is still a bit blackish. A jelly kind of mass came out but I am not sure why my abdomen was paining for two hours. I can get the pregnancy test done in a few days.

If you have got normal periods now then do not worry. Get the test done whenever you can. If the bleeding is heavy or with clots or does not stop in three to four days then urine pregnancy test is must and also ultrasound will be required

If possible get a blood test done at home as some labs provide option for home collection, and get serum beta hCG levels done.This also helps In ruling out pregnancy and is a better test than urine preganancy test.
