Hi doctor,

I am a 20-year-old male who has caught chlamydia or gonorrhea most likely. I feel light burning when urinating, and I get small amounts of translucent/white discharge from my penis. I have gotten tested for gonorrhea and a lot of other sexually transmitted infections, and they all came negative. I did not get tested for chlamydia as the test was not available where I am. I have gone to a doctor, and I was prescribed - a 1g injection of Ceftriaxone, 1g of Azithromycin tablets, and 1g of Ciprofloxacin hydrochloride tablets. I took all of these 15 days ago. After five days, my symptoms did not improve, and the doctor told me to drink 1g of Azithromycin and have another 1g injection of Ceftriaxone. Ten days have passed since the second injection, and I still have my symptoms. Although the burning while urinating has reduced, the discharge has also reduced a little. I am scared because I feel like the pain while urinating has gotten a tiny bit worse from yesterday or two days ago, or at least it has not gotten any better. I contacted my doctor again, and he first said to repeat the antibiotics, and then he changed his mind and said maybe I should take Pyridium for two weeks instead. After that, I should repeat the antibiotics if I do not feel better. I do not like how he is unsure about his decision, and I wanted to consult a different doctor to see if you agree with my doctor's decision. I have a friend who is studying urology, and he told me that the usual treatment for chlamydia is a Doxycycline course for one week. I feel very stressed, and I do not know what I should do, who I should believe, and if I should be worried that the infection has not gotten away already.

Firstly, the positive rate of all STD (sexually transmitted infections) investigations is low, so having negative reports does not mean that infection is not present. Both advice given to you regarding Doxycycline and Azithromycin is correct. Both regimens are used for treatment, and Ceftriaxone is used in injection form. These infections sometimes do take time to improve and have recurring nature also. You will need to repeat antibiotics if symptoms persist or reoccur. A prostate infection called prostatitis also presents similarly, and its treatment is also the same. Stress increases the symptoms of prostatitis, which may be true in your case. Since your burning and discharge have gone down, that is a good indication that the infection has subsided. The best way to tackle such infection is to do a urine and urethral discharge culture and start antibiotics according to the culture report.

I think I will go with Doxycycline treatment and was wondering what kind of treatment plan would be best in my case, considering my weight and my situation. Also, does Doxycycline treat prostatitis?

Yes, Doxycycline is used for the treatment of prostatitis. I suggest that whichever treatment you choose, always do it under the supervision of a local urologist. In adults, weight is usually not the criteria for deciding on treatment. It is very important to avoid all alcoholic beverages till you are on Doxycycline treatment.
