Hi doctor,

I recently had a UTI last month that I treated with Trimethoprim suspension of 20 ml and 400 mg daily for five days. I saw improvement for the last and next two days but noticed some symptoms were back. So I began taking a natural powder supplement called D-Mannose. I took four small teaspoons of the powder with water daily.

This made me go to the toilet more often, and the stools were a lot softer with an odor I never had. It was mentioned on the supplement that this could happen, and I took it for a few days only.

Since finishing the antibiotics and this powder, I have noticed my vagina and anus burning intensely every day continuously for over a week. I also have some wriggling feeling in my butt, but I do not notice anything in the stool. Nevertheless, it burns, and it hurts to pass stools.

I also noticed both my vagina and butt burn daily, but it also feels wet, and when wiping myself, there seem to be some very slight brown marks, as if I did not clean them properly.

After using the internet, I am worried I have done some damage or will need surgery. So I tried not to read things and instead ask you for your opinion. It has only been a little more than a week, but no improvement.

A nurse has looked and said both the vagina and butt seem a little red and a bit inflamed. However, when I look closely in the mirror, the butt looks slightly like a bit of the skin line, a line almost and a tiny lump.

I am worried it is bad and would not heal. I have also done some vaginal swabs to see if I have got some yeast or fungal infection after these antibiotics, as I also have vaginal burning. But since I took that powder, I have had bad stools (not diarrhea), but they were very soft and constant for a few days. So I am worried I have caused more problems. However, I have stopped all medicines and supplements now.

Do you think I can heal even after having symptoms for over a week? And any advice in the meantime? I have also been using Canesten thrush cream for that area for two days and have not noticed much difference yet.

Thank you for the reply.

Yes, the burning is constant, but it does not stop. There is more discomfort when going for a stool and wiping too. It is very sensitive when washing. I have never had this before, only once or twice, and very mild when having a bad stool. The fact this lingers is worrying me. It has been more than a week. The nurse said she feels I also have some form of yeast infection since coming off antibiotics.

It also feels slightly wet down there and has a constant burning sensation. I feel it more when I am lying in bed. When I wipe, I do not notice blood. I am on my period at the moment. There may have been a little blood before, but none now, and when I wipe, I do it often to ensure I am clean. But I still notice some markings as if I did not clean properly, which has not happened earlier.

It has been about a week, doctor, but the symptoms are always there. Do you feel this is curable and does not need surgery? I will take a look at what you suggest.

I read your query and can understand your concern.

I will try to answer them one by one for you.

1) This seems to be an anal fissure or anal mucosa erosions and inflammation, as I mentioned earlier. This will improve with some precautions and medications, as I suggested.

2) Time duration: One week or so is not that much time, especially when you are not using the specific treatment for the cause. So yes, you are right, sometimes, these problems are minor and resolve on their own within a couple of days, but sometimes you need some medical treatment like this time.

3) Yeasts are normal flora of the vagina as well. However, with antibiotics use, if you feel or your nurse says that you might have an overgrowth of yeast, so do not worry. Instead, you can use probiotics (good health bacteria) for a while to replace your normal flora. For this, you can use a commercially available probiotics sachet like the Ecotec sachet once daily, or I will suggest daily yogurt intake (a natural source of good bacteria in your diet).

4) Whenever there is inflammation, you may feel wet because of over secretions. This should improve with treatment.

5) Surgery is only recommended for refractory causes with severe symptoms, bleeding, or unbearable pain. That is also after medical trial failure, up to four to six weeks. But you do not have any of the above issues. I hope this will help you out.
