You need to do some tests to get the diagnosis. Some kind of inflammation of the urethra will not have any known cause and it responds variably to medications. You need to do tests for infections like gonococcal infection, chlamydia, syphilis, HIV (human immunodeficiency virus), and prostate infection. You need to do a urethral swab for gonococcal and chlamydia, a blood test for syphilis, and semen culture. After doing the tests, you can take tablet Doxycycline 100 mg twice daily for 14 days. This medication is effective for urethritis and lower genital tract infections. After getting the results, kindly share the reports. So that, treatment can be given accordingly.

Your semen culture shows a positive result which means that you have bacterial prostatitis. You can take tablet Cefuroxime 400 mg twice daily for four weeks. If you are taking Doxycycline, then you can continue taking it for four weeks. Regarding semen culture showing sperm counts, it is not possible to show fertility. You need to do microscopic analysis for fertility purposes after completing the treatment of prostatitis. After completing the treatment, you can do a semen analysis.
