Hello doctor,

I am a 29-year-old male. I have pain in the cervical nerve causing numbness in left-hand fingers. The same is now radiating to the right hand as well. For the last one to two months, I face numbness on the left cheek, irritation in the left ear (waterlogged), headache left side, and pain in the upper neck backside (left). I had done the MRI and have the reports as well.

Pain in the neck region radiating to the left arm and right arm with numbness in the left-hand fingers can be due to these cervical spine changes detected in MRI (magnetic resonance imaging) cervical spine. Also, we need to get NCV (nerve conduction velocity test) for bilateral upper limbs.

You can start with tablet Maxgalin ER 75 mg HS for two weeks. Along with the use of a soft cervical collar. Avoid prolonged use of cellphones and forward neck flexion and weight lifting. Avoid the use of pillows while sleeping. If you need to use, use thin and hard one (not the soft one).

But numbness in the left cheek, irritation in the left ear (waterlogged), and headache on the left side is not fully explained by these cervical spine findings.

For these symptoms, I suggest first meet an ENT specialist to rule out any ear problem. If nothing is found, may require to visit a neurologist for a physical consultation.

Investigations to be done

NCV bilateral upper limbs. Serum vitamin B12. Serum vitamin D3. T3, T4, TSH (thyroid-stimulating hormone).

Differential diagnosis

Cervical PIVD (prolapsed intravertebral disc) with radicular pain.

Treatment plan

Tablet Maxgalin ER (Pregabalin extended-release) 75 mg HS for two weeks.

Preventive measures

Use a soft cervical collar. Avoid prolonged use of cell phone and forward neck flexion/ weight lifting. Avoid pillows while sleeping. If need to use, use thin and hard one (not the soft one).

Regarding follow up

Consult an ENT specialist. You may need to visit a Neurologist physically for clinical examination.
