Hello doctor,

I need to know the heart condition of my relative. I have her ECG. She felt pricking pain two weeks before. She now takes a BP tablet (Telmesat). I am attaching her reports. Her BP is 150/90 mmHg. Kindly give your opinion.

Her ECG (electrocardiography) is normal (attachments removed to protect the patient's identity). Is her BP (blood pressure) still not under control using Telmisartan? Ideally, BP should be less than 130/85 mmHg for good results. So if it is not under control using Telmisartan alone, then we need to change it to another medication to achieve good control. Now regarding her chest pain, firstly, pricking pain may not be cardiac in origin. Cardiac pain is usually dull aching, cannot be localized with a finger, and continuously persists for few minutes to few hours. Also, it increases exertion and reduces with rest. The pain occurs intermittently at rest and persists for few seconds are usually not cardiac. If pain specifically occurs on deep breathing, then it may be related to infection rather than the heart. Now at this age group with high BP, it is worthwhile to go for other routine cardiac investigations, to be sure, like echocardiogram and TMT (treadmill test), lipid profile, and random blood sugar.
